 Who is eligible for CHDP services?

  • Medi-Cal recipients from birth to age 21, who are not members of IEHP or Molina Health plans
  • Non-Medi-Cal eligible children and youth from birth to age 19, whose family income is equal to or less than 200 percent of the federal poverty level

      • Health and developmental history
      • Physical examination
      • Nutritional assessment 
      • Immunizations
      • Vision testing
      • Tuberculin skin testing
      • Hearing testing
      • Laboratory testing
      • Health education and anticipatory guidance
